If your child is on medication from the doctor then please complete the medical form below.  You will need to complete this form and bring it to the school office along with the prescription medicine for us to administer to your child.

Please complete the separate form below if your child is recovering from an injury, prior to them returning to school.

Please do not send your child to school if they are unwell, for them, their peers and our staff. 

Please note our Allergy Safety Lead is Mrs Moreland, this is in-line with new legislation where every school should have a nominated lead. The named Governor Mrs Langmead.
